For a society so fixated with improvement — of our homes, our morning routines, our bodies, our finances — it is remarkable in what limited ways we define progress.

Geometry was not a subject I found particularly compelling during my school days, but just this week I found myself reaching for the term “sine wave” as a patient and I discussed the difference between his expectations of progress and the nonlinear path progress often takes. I wanted him to understand that, like the sine wave, progress oscillates and undulates with unpredictable frequency and magnitude. We all want to put in X amount of effort over Y amount of time and watch as our progress take the shape of a gradual, upward course. That perspective is very human, but it’s also very far from the truth.

The truth is, when it comes to the trajectory of recovery and healing, progress rarely looks the way we expect.

For a patient of mine a few years back, progress looked like this: her in the fetal position on our office floor, shaking and crying after a mindful relaxation exercise sent her into a paradoxical anxiety vortex. She’d been through significant childhood trauma, and as a result, she’d long been stuck in a frozen physiological state. Those of you who do trauma work are more than familiar with this type of response, which is a form of nervous system release. The patient was finally, years later, discharging the “fight or flight” energy — the neurochemical bundles that had been trapped in her body since those early life events.

In other words, even on that floor in the throes of anxiety and discomfort, the patient was making progress. Though, as I’m sure you can imagine, it didn’t feel that way to her.

In my first few years as a psychiatrist, it wouldn’t have felt like progress to me either. It took experience, supervision, and both personal and professional work for me to trust enough in the process, and in my own abilities, to recognize progress where others may not — like in the relapse that prompts a patient to finally accept and name their alcohol use disorder, or the depression that feels — for the first time — manageable and short-lived.

Similarly, it took time and experience for that patient to realize that her setbacks, slow downs, regressions and stalls — even those that left her in tears on the floor — were ultimately moving her closer to her healthy, authentic self. She’s in a loving relationship now, and has a full-time job. I see her every three months, and each time I’m impressed with her continued growth.

She never would have believed me, when we first starting working together, that her recovery could take her to where she is now — happy, healthy, and thriving. Just as she didn’t believe me, when she got up off that floor, and I told her that it was a step in the right direction.

I believe that it is our role, as an integrative team, to hold this expanded vision of progress….not only for our patients, but also for ourselves.

Panic Disorder

Many people have trouble distinguishing between a panic attack and an anxiety attack. Panic emerges from a state of anxiety that’s been triggered by an acute event that can feel critically dangerous. People suffering from panic attacks may even rush to the ER in fear that they are dying because of a number of physical symptoms, such as shortness of breath, palpitations, tingling or numbness, hyperventilation, nausea, and lightheadedness. When attacks are repeated and severe, they may cross into a disorder that alters behavior to avoid triggers, such as crowds or an airplane that are difficult to escape from, out of fear of experiencing another attack.

Post-Traumatic Stress Disorder

One of the types of anxiety disorder most commonly associated with war veterans, post-traumatic stress disorder (PTSD) can affect anyone who has suffered a life-threatening or traumatic event. It’s characterized as a sudden, relentless panic attack that sends someone into fight-or-flight mode. PTSD regularly predisposes sufferers to hyper-vigilance. They may be easily startled by loud noises or touch, have poor concentration, and may withdraw from others. They will also avoid situations that remind them of the inciting event.

Obsessive-Compulsive Disorder

Popular culture has practically turned obsessive-compulsive disorder (OCD) into a catchall for someone with ritualized tendencies. Images of someone locking the door three times before leaving home or avoiding cracks on a sidewalk have been appropriated comedically in film and television, but OCD should be taken very seriously. OCD stands out from other types of anxiety disorder for its repetitious nature, but underlying the repetition of actions is a pattern of thought that can be extremely damaging. Intrusive thoughts and unwanted behaviors that seem uncontrollable, along with obsessions surrounding fear or worry and undesirable compulsions like chronic hand-washing or counting as a coping mechanism, can seriously disrupt daily life.

Social Anxiety Disorder

Social anxiety disorder (SAD) stands out from other types of anxiety disorder for instilling a debilitating fear of being seen negatively by one’s peers or humiliated in public. This crippling anxiety may lead to excessive shyness and, in extreme situations, avoidance of social situations altogether. It may also create problems with delivering presentations or performing creatively. People suffering from SAD may become reclusive and isolated, an extremely detrimental pattern for mental and physical wellbeing.

Is Depression Hereditary?

Although there are many types of depression, the one that affects many millions of Americans and that has experienced the most coverage in popular culture is clinical depression, also referred to as major depressive disorder (MDD). Among the different forms of depression, MDD is more often shared by siblings, children, and other close relatives. Studies have shown that having a relative who struggles with depression makes an individual almost five times likelier to develop it. The scientific community is clear on the fact that depression is hereditary. But why? Is it nature, nurture, or a combination of the two?

Genetics & Environment

In a recent research effort, a team from the UK isolated chromosome 3p25-26 in more than 800 families who exhibited recurrent major depression. Their results were corroborated by another research group, amounting to “the first report of a genome-wide significant locus for depression that also has an independent genome-wide significant replication.” At the same time, the authors of the paper are careful to stress that both genetic and environmental factors play a hand in whether or not a predisposition for something like depression expresses itself in a person’s lifetime. Growing up in a household with someone whose depression renders him bedridden, for example, may influence what a child perceives as normal behavior. A child may also unknowingly mimic depressive behaviors in a parental figure as a natural part of her development.

While being exposed to depression and depressive behavior can further predispose a child to develop MDD or a similar disorder later in life, other dynamics at home may play an equally critical role. Although scientists have yet to understand why, exactly, the key may lie in further work on epigenetics, which investigates how an environment can influence whether or not certain genes turn on or off.
